Share Your Smile and
Help the Community!
Our practice continues to grow because of the referrals of caring patients like you. That's why we have established our Care Enough to Share program, to thank you for the opportunity to be of service, and to provide a unique way for all of us to give back to the community. We trust the care you received from Dr. Foley and her staff will inspire you to share us with your friends and family. And now, through the Care Enough to Share program, when you do, we will be honored to make a $50 donation in your name to one of the following local charities:
- Happy Tales Humane Society
- The Boys & Girls Club of Middle Tennessee
- The Interfaith Dental Clinic
- Vanderbilt Children's Hospital
Here's how the Care Enough to Share program works: - Send us an email, or call our office at 615.591.0294 to request your copy; or download one by clicking here.
- Write your name and address on the Share My Smile card.
- Detach the form and give it to a relative, friend, neighbor, or co-worker who has never been to our practice.
- When they schedule a visit, and give us your Share My Smile card, we'll donate $50 to the charity you designated on your card. And, we'll send you an e-mail or give you a call to let you know the donation has been made in you and your friend's behalf!
- There is no limit on how many donations we will make, so if you'd like, you may photocopy your Share My Smile card, or let us know and we'll be glad to send you more.
We are thrilled to be able to do this on your behalf, giving back in a special way that benefits all of us here in Middle Tennessee! We look forward to serving you, your family and friends again soon! |
